Occupational Health and Safety Form for Animal Handlers and Visitors (PDF) This form must be carefully read and completed by any individual that may have indirect exposure to or direct contact with UC research animals and is not listed on an approved UC IACUC protocol (e.g. visiting scientists, individuals attending brief training courses, contractors, tradesmen). The individual must review and sign this form prior to exposure/contact and complete any species-specific training required by the Principal Investigator or LAMS for the animals and/or pathogens with which they may be in contact. A UC Sponsor of the individual must also sign the form. It is recommended to submit a completed copy of this form to the IACUC Office to ensure the integrity of conducted research.
