DEA Form 106 (Report of Theft or Loss of Controlled Substances) (Opens in new window)

DEA Form 106 is used to report any theft or loss of controlled substances. It must be submitted within 45 days of discovery.

Accidental drug breakage, spills, or other accidental losses of controlled substances do not need to be reported to the DEA. However, this must be documented and witnessed by the registrant or authorized agent on the related usage or administration log.
